Abstract

The trends of pregnancy by using third party sperms are being popular to deal with the challenges of infertility that raise different ethical questions. Keeping into consideration of ethical points, it is important to examine the psycho-social wellbeing and rights of the donor-conceived child. The aim of this study was to identify a scenario related to some ethical issue and analyze and comment on it. The scenario was analysed in light of ethics, social, legal and Muslim context. The analysis revealed that the sperm donor should ponder over the wellbeing of the unborn child before sperm donation and parents should respect the child's right before taking a decision. It is recommended that healthcare professionals should consider the entire dimensions of the patient's contexts before proposing such controversial treatment modalities.

Abstract

Hepatitis C is a major public health problem affecting 3% of the population around the world. However, worldwide prevalence of hepatitis C in pregnant women is variable. In Pakistan alone prevalence of hepatitis C in pregnant women varies from 0.7% to 36%. Unfortunately, a rising prevalence of HCV in pregnant women is making them vulnerable to complications. Therefore, the purpose of this analysis is to estimate a prevalence rate that represents the actual burden of hepatitis C in pregnant women; by pooling the prevalence of hepatitis C in pregnant women reported in 17 studies from Pakistan. Pakmedinet, PubMed through the National Center for Biotechnology Information (NCBI), MEDLINE and Google Scholar search engines were employed for literature search. Studies published in the English language from the year 2001 and onwards have been included in this review. Since there is a paucity of studies related to this topic, we did not exclude any published literature fulfilling inclusion criteria. The quality of each study was assessed using the Newcastle–Ottawa Scale. Paper review was done by two reviewers with data extracted and compiled in MS Excel. The registration number of the study is CRD42016042341. A total of 17 studies from Pakistan reported prevalence of hepatitis C in pregnant women; 3 each were conducted in Islamabad and Khyber Pakhtunkhwa respectively, 5 in Punjab and 6 studied were conducted in Sindh province. From 17 studies, we pooled 124635 pregnant women screened for hepatitis C and of them, 8342 women were found to be positive for Anti-HCV. The overall prevalence of hepatitis in pregnant women of Pakistan was 6.7% (95% CI: 6.5-6.8) while its range varied from 0.7% to 36%. In Pakistan, the overall prevalence of hepatitis C in pregnant women was 6.7%. Nonetheless these findings cannot be disregarded and should be used by Policymakers for considering the antenatal screening of hepatitis C in pregnant women.

Abstract

Matching nursing resources with patient needs and acuity while maintaining patient safety and quality of care has become increasingly difficult due to the increase in nursing shortage. Nurses have growing concerns about equity and satisfaction regarding patient assignments. The purpose of this project was to discover the effects of utilizing an acuity tool that interfaces with the electronic health record (EHR) to accurately assess patient acuity and balance the nurse-patient workload. A search plan was developed to identify keywords within several article databases. Existing literature identified evidence on how an electronic acuity-based staffing tool balances nurse-to-patient workload and allows for consistent, high-quality patient care while managing healthcare costs. An acuity measurement tool interfaced with the EHR can be an excellent resource for optimizing nurse staffing and workload. Electronically predictinga patient’s acuity provides a powerful tool for healthcare managers to allocate resources. Such estimations and predictions on patient’s acuity can be produced from vast amounts of electronic healthcare data, information technology, and computational intelligence techniques. Using tactical decision-making and resource allocation with different mathematical optimization models will support acuity predictions and help to balance nurse workloads.

Abstract

Nurses, the mainstay of the healthcare system, occupy a significant position at all tiers of health care. Inadequate number of nursing staff affects the quality of nursing care and also elevate the burden of workload on existing staff. This study aimed to assess factors influencing nurses’ job satisfaction and retention in a public sector tertiary care hospital in Pakistan. An exploratory qualitative study was used to explore nurses’ perception of job satisfaction at work. Study was carried out during July to September, 2016 at public sector tertiary care hospital of Pakistan using purposive sampling technique. Study participants were nurses, providing direct patient care and head nurses in 10 different units of hospital. Focus Group Discussions were conducted with staff nurses from eleven different clinical units; In-depth Interviews with head nurses and Human Resource/Administration personnel. Human Resource policy related documents were also reviewed. Lack of career growth prospects and in-service education and trainings, professional discrimination between physicians and nurses, high work load and shortage of staff were the important factors indicated by participants for job dissatisfaction. Nurses also expect recognition for their work and respect from their co-workers. Job satisfaction among nurses can potentially increase quality of nursing care. The study highlighted the need to implement fair and equitable policies for nursing workforce at hospital. Simple measures for better management practices could significantly improve the working conditions and hence retention of nurses in the longer run.

Abstract

The events happening in the early hours of life are very effective on the survival and development of the child, and the separation of the newborn from the mother after birth, even in the short term, can disrupt successful breastfeeding at this important time. The current study aimed to determine the effect of skin-to-skin contact of mother and newborn immediately after the cesarean section on breastfeeding choice at the time of discharge. This randomized controlled interventional study was performed on 80 mothers and newborns after the cesarean section with spinal anesthesia in two groups of skin-to-skin contact and routine care. The type of feeding selected by the mother was asked at the time of admission and discharge, and it was compared between the two groups. Data were analyzed using SPSS 22, logistic regression, Chi-square and Fisher tests. The results showed that the selection of breastfeeding at the time of discharge in the skin-to-skin contact group was slightly higher than that of the routine care group, but there was no statistically significant difference between the two groups regarding the significance level of the study (p-value >0.05),. 96.3% of the mothers of the skin contact group reported that the skin-to-skin contact was effective in choosing exclusive breastfeeding for their newborns. The results of this study show that although a high percentage of mothers tends to breastfeed their newborns, in practice almost half of them do not succeed in breastfeeding their newborns exclusively. According to the results of the study, and since the selection of breastfeeding in the skin contact group was at a higher level, skin-to-skin contact did not have a significant effect on the selection of breastfeeding. It seems that a study with a larger sample size will produce more positive results.

Abstract

Preoperative assessment plays a vital role in all surgeries that are to be performed under general anesthesia. It helps in the preparation of the patient and optimization of risk factors before surgery, involvement of multidisciplinary teams to avoid complications, decrease length of stay, and reduce the delays in surgery. The aim of this quality research work was to improve preoperative anesthesia assessment compliances of patients undergoing elective orthopedic surgeries. A retrospective review of data was conducted for orthopedic services in a tertiary care hospital for a period from January to June 2017 of all patients planned for elective surgeries. The data analysis showed only 31% compliances. In order to enhance these compliances, a preoperative nurse was hired in the service and she functioned closely with the surgeons. After interventions, the preoperative compliances increased from 31 to 81% in the next three months from July to September 2017. The targeted benchmark to achieve maximum compliances is up to 95 to 100%. Preoperative optimization not only benefits in early recognition of risk factors, but it also contributes towards positive clinical outcomes of patients after surgery. Moreover, it promotes speedy recovery and decreases the length of hospital stay.

Abstract

Resilience is a process of recovery, an individual's capability and a consequence of successfully adapting despite adverse circumstances or suffering to personal wellbeing and protection. The aim of this paper is to define resilience as a concept, construct, process, and outcome during adolescence. A review of the literature on adolescent resilience was conducted and analyzed using resilience as a concept, construct, process, and outcome during adolescence. Adolescence is considered as a phase of rapid development with consequences such as the adoption of risky behaviors. Promoting resilience in adolescents require protective processes at three different levels which include individual level, family level, and community level interventions. These factors promote individual's positive coping and fast recovery during adversities, which results in improved mental and physical health, such as enhanced self-esteem, hopefulness, problem-solving, and better confidence in facing challenges. Nurses must be mindful of the developmental stages, coping styles, level of stress, and risk behaviors in order to understand their level of resilience. Understanding resilience would assist nurses in practicing holistic nursing care to adolescents.

Abstract

Giving birth is a beautiful experience and greatest achievement for a woman. And also everything is needed to be accomplished in appropriate time to avoid consequences. As like in preterm labor, where the labor occurs before the 37 th completed weeks is associated with various maternal and neonatal complications. As per statistics, about 28% of the neonatal death occurs due to preterm birth and the prevalence of preterm labor worldwide is around 11.5%. There are a lot many causes to it including preterm rupture of membranes (30%), urinary tract infections during pregnancy (35.8%), history of abortion (19.9%), and unplanned pregnancy (18.1%). But fortunately there are preventive measures like daily uterine activity monitoring, adequate bed rest and hydration, use of polar bear position, and vitamin C supplementation. If those fails, pharmacological treatment measures (tocolytics, Prostaglandin Inhibitors and Calcium Channel Blockers, Antibiotic therapy) can be used. However, the treatment and preventive measures is still a topic of debate. Also, nurses has a pivotal role in handling mothers with consequences.

Abstract

The first hours after birth are a critical period to develop the mother-newborn attachment and to reduce the anxiety related to the baby. Cesarean section and subsequent separation of mother and baby can increase the disorders resulted from this separation. Thus, the present study aimed to investigate the effect of early skin to skin contact on maternal attachment behaviors in neonates after cesarean section in a hospital in South East of Iran. This is a randomized controlled trial study, which was done on 80 mothers and newborns after cesarean section with spinal anesthesia in a hospital in South East of Iran operating room. The research samples were assigned into two groups including; skin to skin contact group and the routine care group. In the intervention group immediately after the birth, the baby was placed in prone position on the mother's chest, and in the routine care group, the baby was placed under radiant warmer just immediately after the birth. Afterward, the data were gathered by using checklists of Avant mother attachment behaviors. Data analysis was done by SPSS version 22. The results showed that there is no difference between both groups in the demographic variables. A significant difference was found between both groups in the mean scores of emotional behaviors (p<0/0001) and caretaking behaviors (p<0/0001). However, no significant difference was found between the two groups in the close contact behaviors (p=0.22). According to the results of this study, mothers who had skin-to-skin contact with their baby showed more emotional and caring behaviors. Therefore, in cesarean sections, due to the problems with starting breastfeeding as well as long separation between mother and baby after surgery, skin-to-skin contact is recommended after birth as an inexpensive and easy method.

Abstract

Preterm birth is considered as a stressful situation for parents. A training program for the preparation of the mothers is essential at the time of discharge due to more vulnerability of these infants compared to term infants. Mothers of these children are worried about caring for their child after discharge. In Iran, there is no follow-up regarding the status of preterm infants after discharge. This study aimed to assess the effect of implementation of discharge planning on hope and satisfaction of mothers with preterm newborns in Afzalipour Hospital in Kerman. In this clinical trial study, the population included 100 mothers of preterm newborns hospitalized in NICU in Afzalipour Hospital of Kerman in 2016. The mothers were randomly assigned to the control and intervention groups. Discharge planning was implemented for mothers that consisted of training programs about caring at home in the intervention group while the control group received routine training that consisted of receiving the booklet. Both groups completed hope and satisfaction questionnaires before and after the intervention. The questionnaire consisted of three parts: Demographic information, parental satisfaction, and Miller hope scale. The data were analyzed by Man Whitney U and Wilcoxon as well as SPSS 19. The results of this study showed that there was a significant difference in the mean scores of satisfaction between intervention (209±8.3) and the control (181.82±21.97) groups after the implementation of discharge planning (P<0.001). Also, according to Mann Whitney U test statistically, there was a significant difference in the mean scores of hope between intervention (277/02±10.28) and control groups (217.6±28.39) after the implementation of discharge planning (P<0.001). There was no statistically significant relationship among satisfaction, hope, and any demographic variables according to Spearman test (p>0.05).

Abstract

An individual undergoes through various psychological changes during his life. These changes sometimes result in disparities hence, life loses its state of equilibrium. This gives rise to risk taking behaviors like suicide. The literature review has been conducted to identify and highlight the causes of suicide, and some strategies to reduce its rate in order to promote individuals' physical and mental health. Various databases and search engines, ranging from 1998 to 2013, have been used to conduct a detailed analysis of the problem. Epidemiological studies show that suicide has taken a heavy toll. In east, suicidal behaviors are misinterpreted as honorable martyrdoms (Vaknin, 2013). Moreover, teenage suicide is the third leading cause of death among adolescents (Shain, 2007). These findings suggest that an instant attention is required towards the issue. Various socioeconomic, psychiatric, and personality disorders and sometimes the mixture of these are responsible behind its existing trends. Therefore, the resolution of the problem requires a multidimensional approach. Suicide is considered as a failure of coping mechanism. Since, it negatively influences health, it becomes important to reduce its trends. A collaborative effort of health professionals, society, and governmental organizations is required to hamper its growth.

Abstract

The women of today face not only diseases of genetic origin, but more importantly, diseases that arise from the poor habits that society has developed. As former Prime Minister Jawaharlal Nehru said, “You can tell the condition of a nation by looking at the status of its women” (Chankapa et al., 2011). The study aimed to investigate knowledge and attitude about cervical cancer and HPV vaccines among the age group between 14 to 30 years in urban community of Maharashtra. The study adopted a quasi-experimental “one group” with 'pre-test' and 'post-test' approach with convenient sampling technique. A sample of 50 women of age group 14 to 30 years was selected. The effect of structured teaching programme consisting of cervical cancer and HPV vaccine are evaluated. Descriptive statistics used and pre-test, post-test analysis was done. Findings: The pre-test, post-test study revealed significant differences seen in knowledge and attitude between the pre-test and post-test. In the pre-test, 44% had good knowledge and 60% agreed in the attitude. Whereas post-test 62% had very good knowledge and strongly agreed in the attitude questions. The results show that there is a difference in knowledge and attitude by 18% and 2%, respectively, which also suggest that a structured teaching programme was effective. The study shows that pre-test scores were increased after giving structured health teaching regarding cervical cancer and HPV vaccine. This increase in post-test scores suggests that education may increase knowledge of cervical cancer and HPV vaccine.

Abstract

Breast engorgement is a physiological condition during the first two weeks of child birth in which there is lymphatic and vascular congestion and pressure of new milk. It is associated with hard, painful, throbbing, aching, and tender breasts, which may result in women needing analgesia, developing mastitis or temporarily or permanently stopping breastfeeding. The nurse should understand the effect of breast engorgement on the mother and recognize its influence on the baby. One group pre-test - post-test research design was adapted for the study. In view of the nature of the research problem and objectives, a structured questionnaire and structured teaching programme was developed in the initiation of breastfeeding within one hour of birth, composition of breast milk, advantages of breast feeding, techniques and problems of breast of breastfeeding and its management. Sixty primi postnatal mothers were selected by non-probability convenient sampling technique. Structured Knowledge questionnaire was used to collect needed data. After pre-test the structured teaching on prevention and management of breast engorgement was given for 45 minutes. Post-test was administered after five days. The data collected were tabulated and scrutinized by using descriptive and inferential statistics. Results show that 90% of the mothers had an overall inadequate knowledge, 10% of postnatal mothers had moderately adequate knowledge, and none of them had adequate knowledge in pre-test. In post-test 18% had moderately adequate knowledge and 82% of postnatal mothers had adequate knowledge and none of them had inadequate knowledge regarding prevention and management of breast engorgement. The study concludes that the planned teaching programme was effective in improving the knowledge of the postnatal mothers regarding prevention and management of breast engorgement.
